Application Process:

You have 2 options for applying. 

 

#1.  Call us at 616-772-1591 to enroll in person at the school.  It only takes one hour to apply.  This is the quickest & easiest way to enroll.

 

#2.  Contact us by phone or e-mail and request an Application Packet.

  • Follow the instructions on the “Application Checklist” and return the material to us in the pre-addressed envelope provided.
  • Lakeside Academy will review all information for content and requirements.
  • Lakeside Academy will notify you within 5 days of your denial or preliminary acceptance into the program by letter.   
  • Upon acceptance into the program you will need to fill out the enrollment agreement and any loan papers at the school.

The complete cost of the program is $10,950.00. This fee includes the cost of all books required for the course, two uniforms (tops & pants), liability insurance, and other required materials (see enrollment agreement for complete details).  Tuition may be paid in full before the first day of class, or payment plans are also available. 

A diploma as Certified Massage Therapist will be issued upon successful completion of all program requirements.

Application Requirements:

All applicants to the Academy are expected to meet all of the following minimum requirements and must be included with your application:

  1. A completed Application form.

  2. A signed No Harassment Policy.

  3. A completed Release Form for Media Recording.

  4. A completed Personal Information form.

  5. Copy of Birth certificate as proof applicant is age 18 or older at time of program start.

  6. A written request for enrollment which must include an explanation of motivation for training in Massage Therapy (this is provided on the Personal Information form).

  7. Official Grade 12 high school transcript  or GED attesting to successful completion of such.  A copy of your High school diploma may be used if transcrips are not available.

  8. Three original letters of reference may be required. They must be from persons who can attest to your personal character, professional and/or academic skills. They should preferably be, for example instructors, employment supervisors, pastors, colleagues or health care professionals, not personal friends or family members.

  9. Two recent passport size color photos.

  10. A current medical and physical certificate signed and dated by a medical doctor, stating that you are in good health and free from communicable diseases.

  11. A check or money order for $625.00 tuition deposit (includes registration fee).

  12. An interview with the Dean. 


    CANCELATION & REFUND POLICY

If Lakeside Institute L.L.C. rejects or denies an applicant before enrollment, all monies paid by the applicant shall be refunded.  An applicant fee of not more than $25.00 may be retained by Lakeside Academy if the applicant is denied.

 

If an Applicant decides to cancel enrollment at any time before the start of the program, the applicant is entitled to a refund of all monies paid to the school except $100.00.  (The applicant fee of $25.00 plus the registration fee of $75.00, for a total of $100.00 will be retained by Lakeside Institute L.L.C. )

 

All monies including the registration fee are 100% refundable for three (3) business days after signing the registration agreement.  After three business days the registration fee is non-refundable. 

 

Refunds will be issued to the student no later than thirty (30) business days following the institute’s receipt of the student’s written request.  An applicant student not requesting cancellation by the start of the program will be considered an enrolled student.  Once classes have begun, the withdrawal date will be used to calculate refunds or amounts due.  Lakeside Institute L.L.C. requires that all cancellation be made in writing to the school.  Lakeside Institute L.L.C. reserves the right to reschedule or cancel any class or program up to one (1) week in advance of the start date.  Lakeside Institute L.L.C. will refund any monies due the enrolled student within thirty (30) days of the withdrawal date.  Refunds are calculated by Lakeside Institute L.L.C. on a Pro-rata basis.  In case of prolonged illness or accident, death in the family, or other circumstances which make it impractical to complete the program, Lakeside Institute L.L.C. - upon request - shall make a settlement that is reasonable & fair to both parties.
